Case Management Service

Supporting Families Every Step of the Way

Raising a child with autism can be a beautiful and meaningful journey, but it can also come with questions, paperwork, appointments, systems, and decisions that may feel overwhelming.

At Autism Care Therapy (ACT), our Case Management Services are designed to help families navigate these challenges with guidance, organization, and compassionate support.

Our goal is to make the process easier for families so they can focus on what matters most: their child, their family, and their child’s progress.

What Are Case Management Services?

Case Management helps families connect with services, resources, benefits, and support systems that may improve their child’s care and family well-being.

At ACT, our Case Management team works with families to understand their needs, identify available resources, and help coordinate care across different providers, programs, and community supports.

Case Management may support families with:

Benefits and resource navigation
Medicaid and insurance-related guidance
School communication and IEP support
Medical and therapy coordination
Community programs and support groups
Mental health and family support resources
Grant and financial resource opportunities
Parent guidance and problem-solving
Ongoing service planning and follow-up

This service is designed to help families feel more informed, more supported, and less alone.

How Our Case Managers Support Your Family

Our experienced Case Managers work closely with families to provide personalized guidance based on each child’s needs and each family’s situation.

Our Case Management team may assist with resources such as:

Medicaid
Private insurance guidance
SSI resources
LINK benefits
Respite care resources
Family support programs
Other available community-based programs

We help families understand available options and take the next steps with more confidence.

Some families may qualify for grants, financial assistance, or community-based funding opportunities.

Our team can help families:

Identify possible grant opportunities
Understand application requirements
Gather needed documents
Submit or prepare applications when appropriate
Explore resources that may benefit the child and family

The goal is to help families access support that may reduce financial stress and expand care opportunities.

Medical Assistance and Care Coordination

Families often need to coordinate information between doctors, specialists, therapists, schools, and other care providers.

Case Managers may help with:

Obtaining or organizing medical records
Connecting with healthcare providers or specialists
Supporting communication between providers
Helping families prepare for appointments
Coordinating therapy-related documentation when needed

This helps create better communication and smoother care coordination.

Mental Health and Community Connections

Families may benefit from emotional support, community programs, and trusted local resources.

Community programs
Parent support resources
Crisis or urgent support resources when appropriate
Local organizations that serve children and families

Support is not only about services. It is also about helping families feel connected, understood, and supported.

Our Case Management team may support families by helping them:

Understand school-related needs and concerns
Prepare for IEP meetings
Communicate more clearly with school teams
Review goals, services, and accommodations
Identify questions to ask during educational planning
Advocate for appropriate support for their child

The goal is to help parents feel more prepared and confident when working with their child’s school team.

Many children receive support from multiple providers. Case Management helps improve communication and coordination across the care team.

This may include coordination with:

ABA Therapy providers
Speech Therapy providers
Occupational Therapy providers
Medical providers
Mental health professionals
School teams
Community service programs

Better coordination can help reduce confusion and support more consistent care planning.

Recreational and Community Opportunities

Children and families may benefit from recreational, social, and therapeutic activities in the community.

Case Managers can help families explore options such as:

Recreational programs
Adaptive activities
Community events
Social opportunities
Therapeutic programs
Family-friendly local resources

These opportunities can support quality of life, social participation, and family connection.

Parent Coaching and Guidance

Case Managers provide information, guidance, and ongoing support to help parents make informed decisions.

This may include:

Helping families understand available options
Supporting decision-making
Providing practical guidance
Helping families organize next steps
Following up on goals and needs
Encouraging families throughout the process

At ACT, we believe families deserve support that is compassionate, practical, and easy to understand.

Getting Started Is Simple

Case Management Services at Autism Care Therapy are funded through Medicaid, making them accessible to many families who qualify.

The process begins with an assessment called IM+CANS, which stands for Illinois Medicaid Comprehensive Assessment of Needs and Strengths. According to Illinois Healthcare and Family Services, the IM+CANS is a standardized tool that helps assess a person’s needs and strengths and supports care planning and treatment recommendations.

During this process, our team learns more about your child’s strengths, needs, family goals, and support priorities. Based on this assessment, we create a personalized care plan designed to support your child and family.

After the assessment, your family will be connected with a dedicated Case Manager who can meet with you regularly, review your plan, and provide ongoing guidance and support.
