ACT's Case Management team helps Illinois families manage all of it, with services funded through Medicaid for eligible families, so you can focus on what matters most: your child.

What Case Managers help with
Our team supports families with Medicaid, SSI, and benefits navigation, IEP preparation and school communication, and coordination between doctors, therapists, and school teams so information doesn't get lost between providers. We also help identify grant and financial resource opportunities, and connect families to mental health support, community programs, and recreational resources — because support isn't only about paperwork, it's also about helping families feel less alone.
How it starts
Case Management begins with IM+CANS (Illinois Medicaid Comprehensive Assessment of Needs and Strengths), a standardized evaluation that helps us understand your child's and family's needs. From there, you're matched with a dedicated Case Manager who builds a personalized care plan and checks in regularly as your family's needs evolve.
Currently available at: Lombard, Aurora, Bloomingdale, and Kankakee clinics. Availability depends on location, eligibility, and staffing.
